Daily Weight-Based Dose Regimen
– Initial Dose: ≤0.006 mg/kg/day SC
– Dose may be increased up to a maximum of 0.025 mg/kg/day in patients ≤35 years, or up to a maximum of 0.0125 mg/kg/day in patients >35 years
Weekly Weight-Based Dose Regimen
– ≤0.04 mg/kg SC divided into equal doses 6 to 7 days per week
– Dose may be increased at 4- to 8-week intervals to a maximum of 0.08 mg/kg/week

SC Administration
– For SC administration only
– Do not inject IV
– Do not shake
– Rotate administration sites to avoid tissue atrophy
Reconstitution
– Reconstitute with provided diluent
– Swirl gently; do not shake.
– Consult the instructions provided with the reconstitution device.
Storage
– Store intact vials at 2°C to 8°C (36°F to 46°F)
– Do not freeze
– Protect from light
– Following reconstitution, store under refrigeration for up to 3 weeks
– Store vial in carton to protect from light.
– May be stored ≤25°C (77°F) for up to 3 months after dispensing
– Store in original carton to protect from light
